Medical Release: In the event of an accident or serious illness in which the parent/guardian or designated person cannot be reached, I give permission for Awana staff to arrange for immediate treatment. This authority is granted after reasonable effort has been made to reach me by phone. I will not hold Mt. Pleasant Baptist Church, their staff, administration, or workers, liable for any injury to or loss of possessions by the above minor during any scheduled Awana meeting or special event. I, the parent/guardian, will assume responsibility for any fees/costs related to arranged medical treatment.

Photo Release: Photographs are sometimes taken of the church’s Awana ministry activities for publicity and promotional purposes, which include, but are not limited to, church presentations, the church’s web site and social media, brochures and newsletters.
By checking Yes below, I, the parent/guardian, am granting Mt. Pleasant Baptist Church permission to use photographs of the above minor child for the reasons listed, in whole or in part, without further consideration, for as long and often as it should desire. I understand that should my child’s photographs be used online that they will be available for download.
I do hereby agree to hold harmless Mt. Pleasant Baptist Church, its leaders and workers, from any and all liability and damages that may arise from the use of my child’s photographs as indicated above.
